This Cyber Security Incident Reporting Form is intended solely for Singapore-based Cigna Healthcare members. If you are located outside Singapore, please contact your local Cigna customer service or support channel for assistance.
If you have identified any of these issues, we would appreciate
your assistance to report it in the form below. This can include:
- Fraud or scam incidents that involve attempted theft of Cigna account or any personal information, including impersonation of Cigna
- Suspected unauthorised access or compromised access of Cigna member accounts
